All births and deaths occurring within Ohio can be obtained through our department. Certified copies of certificates can be obtained for a fee of $22.00. Burial permits are also issued through this department along with paternity affidavits and genealogy searches.
To Obtain a Birth or Death Certificate:
You may also request certified copies by mail. Simply download a copy of our Mail Request Application by clicking below. Complete the application and mail it to us, along with the $22.00 fee and a self addressed stamped envelope.
Our office hours are Monday - Thursday 8a - 4p (close Noon-1p for lunch).
Payment:
We accept cash, check, money order, Visa, Mastercard, and Discover for payment. Please note on credit card payments there will be an additional $2.00 convenience fee for orders taken by phone.
