Health Department - Vital Statistics

All births and deaths occurring within our jurisdiction since 1908 are on file with our department. Certified copies of certificates can be obtained for a fee of $22.00. Burial permits are also issued through this department along with paternity affidavits and genealogy searches.

You may also request certified copies by mail. Simply download a copy of our Mail Request Application by clicking below. Complete the application and mail it to us, along with the $22.00 fee and a self addressed stamped envelope.

We accept cash, check, money order, Visa or Mastercard for payment. Please note on credit card payments there will be an additional $2.00 convenience fee for orders taken by phone.
